Advanced Health members have access to all medical, dental, behavioral health, and transportation benefits covered under the Oregon Health Plan (see member handbook). This includes doctor visits, prescriptions and medical equipment, dental and eye care, hospital stays and mental health and addiction services.
Free transportation is also available to get you to medical visits.

If you would like a printed copy of our provider directory, member handbook, or any other information from the website, it is available to you at no charge and will be provided to you within 5 business days of your request. Everyone has a right to know about Advanced Health’s programs and services. All members have a right to use our programs and services. We give free help when you need it. You can have an OHA approved and certified voice or sign language interpreter at your appointments if you need one. When you call for an appointment, tell your provider’s office that you need an interpreter and for which language.
Some examples of the free help we can give are:

➢ Written materials in other languages
➢ Braille
➢ Large print
➢ Audio and other formats

What is a Health Risk Survey?

A Health Risk Survey is a list of questions we ask you to see how we can meet your needs. It helps us know if you need more information about some of our benefits and services. We ask you these questions every year to see if your needs have changed.

Why should I take a Health Risk Survey?

This is a short survey that should take less than 10 minutes. Taking the survey can help you with your needs. If we find something you need during the survey, we will help you with resources and other information.

How will I get the survey?

We call all new members and each member every year during the month of their birth. If we don’t reach you by phone, we will mail you the survey.

How do I know the caller is from Advanced Health?

An Advanced Health Customer Service staff will call you to complete the survey. They will introduce themselves and ask for your name and birth date to confirm who you are. They will NOT ask for your social security number or any banking information.
