MISSION: Coos Healthier Together is a coalition of community organizations and advocates who promote healthy behaviors and work toward a healthier Coos County community through the development and implementation of the Coos County Community Health Improvement Plan (CHIP).

Coos Community Health Improvement Plan (CHIP)

The CHIP is a work plan that is developed based on the significant health needs of our community identified in the Community Health Assessment (CHA). The Community Health Assessment (CHA) identifies significant health needs facing the community. For example, the assessment may show that Coos County has a high number of children who are overweight (compared to the rest of Oregon). Data gathered in the CHA are county demographics, social, economic, and environmental factors, health, transportation, housing, public safety, education, land use and air quality. Advanced Health conducts an assessment of the community’s health at least every 5 years to help us prioritize our work in the community. We then put that information in our Community Health Improvement Plan (CHIP) and work on ways we can address the issue of overweight children. Each year, Advanced Health produces a Progress Report that shows us how we are doing on the CHIP projects and the work being done by the subcommittees listed below.

Coos Healthier Together Committee Meeting:

The Committee meets the 3rd Thursday, every 2 months, from 2:30pm – 3:30pm.
