Coos Community Health Improvement Plan (CHIP)
The CHIP is a work plan that is developed based on health needs of our community which are identified in the Community Health Assessment (CHA). Data gathered in the CHA are county demographics, social, economic, and environmental factors, health, transportation, housing, public safety, education, land use and air quality. We put this information in our Community Health Improvement Plan (CHIP) and work on ways we can address the issues. Each year, Advanced Health produces a Progress Report that shows us how we are doing on the CHIP projects and the work being done by the subcommittees listed below.
CHIP Committee Meeting: The Committee meets the 3rd Thursday, every month, from 2pm – 3pm.
