HEALTH EQUITY PLAN (HEP)

A Health Equity Plan (HEP) aims to provide the CCO with a clear framework to becoming an organization that values and prioritizes health equity. The framework includes an action plan detailing where the CCO is headed, what it plans to achieve, the methods it will use, and milestones to monitor progress. Advanced Health creates a new HEP annually that shows us how we are doing in the community and internally.
