Community Health Improvement Plan (CHIP) – Curry County

The Community Health Improvement Plan (CHIP) is a work plan that is developed based on the significant health needs of our community identified in the Community Health Assessment (CHA).  For example, the assessment may show that Coos County has a high number of children who are overweight (compared to the rest of Oregon). We then put that information in our Community Health Improvement Plan (CHIP) and work on ways we can address the issue of overweight children. Each year, Advanced Health produces a Progress Report that shows us how we are doing on the CHIP projects.
